Your section save card
Start here: Open the checked answer beside its actual source. Fill the topic, source and confirmed wording first. Record a version and unresolved item only where they affect reuse; keep the master recoverable.
Save the corrected answer and the source that supports it. Use this card for an area you rely on; you do not need to fill the map first.

Update the master first, obtain review, refresh the working copy and re-ask the affected question in a fresh conversation. Archive old approved masters in restricted storage; keep one current active source.
Keep enough to reuse the answer
Record the topic, actual source, confirmed wording, reviewer where needed and open question. For your own project, a corrected interview note and date may be the source; for school policy, use the governing document. Update the saved note when its facts change, then try an affected question again.
